Here's how the Power Form works for your payroll contribution to the OSU Food Drive:
- When you click on the link DocuSign Power Form, your computer or smart phone will prompt you to login with your ONID user name and password.
- Once you're logged in, your browser will go to a page that looks like this:

- Select the 'initiate request' button, and a customized DocuSign form will be sent to your email.
- From there, you can select the amount of your monthly payroll contribution, indicate which competition unit will get credit for your effort, and you're done!
- Every year you'll get an email reminder of your contribution status. You can make changes or cancel at any time.
