Google Doc-OSU Alert Evaluation Form

The OSU Alert Evaluation Form in Google Drive is a shared tool that is used to assess whether an incident poses a serious or ongoing threat to the university community. This form may also be used to assess incidents that do not fall under Clery Act requirements. The OSU Incident Commander will use preformatted and approved emergency messaging templates (Appendix H) to send initial OSU Alerts in response to situations posing an immediate life-safety risk and requiring emergency implementation of protective actions to preserve lives (i.e., emergency notifications). However, depending upon the circumstances, pre-identified templates (Appendix H) may be used in conjunction with this form, such as when developing follow-up messages.

Certain members of the assessment team referenced in Appendix J are responsible for completing assigned sections of the OSU Alert Evaluation Form. The form allows assessment team members to simultaneously document incident details, analyze information, and draft messages. Draft messages are sent to designated team members for further review and approval. Information sections and responsible team members:

Incident Description OSU Incident Commander, or designee (e.g., Clery Act Compliance, Department of Public Safety Dispatch Center, etc.)
  • Provides basic incident information.
  • Contacts OSU AVP for Public Safety/Chief of Police (AVP DPS), or designee (e.g., on-duty patrol supervisor).
Initial Review OSU Incident Commander, or designee (e.g., OSU AVP for Public Safety/Chief of Police, Clery Act Compliance, on-duty patrol supervisor, etc.)
  • Assesses incident for indicators of a serious or ongoing threat.
  • Determines whether the incident meets Clery Act criteria.
  • Determines level of assessment team response.
Incident Details OSU Department of Public Safety
  • Provides investigation details, if needed.
Draft Message Content OSU Incident Commander, or designee (e.g., OSU AVP for Public Safety/Chief of Police, Clery Act Compliance, on-duty patrol supervisor, etc.)
  • Drafts potential message, if necessary.
  • Sends draft message to Approvers (listed below).

  • Emergency Preparedness

  • Reviews and provides message input.
  • Determines mechanism of message transmission.
  • Approves message for transmission.
  • Note: Without regard to the status of the assessment team assembling or making a determination, the IC retains the authority to issue an OSU Alert in accordance with Attachments 2 and 3 of Appendix B.
